If you are a Settlement Class Member, you must submit a Claim Form either through this website on or before August 30, 2023 or, by mail, postmarked no later than August 30, 2023.
You may submit the Claim Form:

Please Note: Any information you provide is confidential and you will remain anonymous. Additionally, we encourage you to submit any/all information and documentation that you have on hand, and the Claims Administrator will determine if sufficient or if any further information is needed.
Please Note: If you are a bank, broker, or other third-party nominee filing on behalf of your clients, please file your Claim on the Nominees page and not through this page.
